This sunflower looks alive and improving, but the long, thin stem and spaced-out growth point most strongly to low light indoors. Sunflowers are heavy light users, so when they do not get enough direct sun they stretch upward, stay skinny, and lean into the light. I do not see a clear sign of rot or a major nutrient problem from this photo.
Give it more direct light
Move it to the brightest possible window or outdoor sun if temperatures allow, and rotate the pot every few days so it does not keep stretching one way.
